Changing to a Treatment Agency in Monmouth County

If you are required to attend an assessment and must switch agencies, please click the following link and choose an agency from the Monmouth County Affiliated Treatment List. Agencies are listed alphabetically by town. Once you have e chosen an agency for the assessment, complete the required paperwork through email or print them out and submit via mail or fax.  

For those that cannot afford treatment services, DUII Funding through the State of New Jersey is available. More information on this funding source and other funding sources, as well as eligibility requirements, can be found at https://nj.gov/humanservices/dmhas/resources/services/treatment/sa_idp.html or speak to the agency. 

Submit Forms Via Email to the IDRC 

Complete the following and click "Submit" and the forms will be directly emailed to our office and to the email you included on the form. On Forms I, II, III, make sure your name and the agency name are on all three forms. Failure to do so will result in the continuation or issuance of a Non-Compliance Report. This could result in jail time and further license suspension.

Once you have returned the paperwork, call the agency to schedule an appointment.

NOTE: Failure to follow through with the referral, including completion of treatment if you are accepted into a program, will find you in Non-Compliance and notice will be sent to the sentencing court and the Division of Motor Vehicles for appropriate action. The Non-Compliance process will not stop and will not be lifted until all IDRC requirements are complete and your file is closed. This could mean further jail time and further license suspension.

Please choose an IDRC-affiliated treatment agency in New Jersey to complete this phase of your treatment as part of your IDRC requirements. Affiliated treatment providers in New Jersey can be found at https://findtreatment.samhsa.gov/

Now that you reside out of the State of New Jersey, you must choose an agency for an assessment in your area. Kindly go to the below website and search for an alcohol and drug treatment agency that offers outpatient services. Self Help Attendance

If you are required to attend Self-Help (AA, NA) as part of your IDRC obligation, a letter will be sent with an Agreement to Participate in an Alcohol or Narcotics Self Help Program. This agreement will outline how often you attend along with when and where you should send the attendance cards each month.
