TEN (10) DAYS BEFORE YOUR HEARING DATE
Proof of Services/Mailing, Affidavit of Personal Service and Notice of Pending Hearing must be submitted to the Monmouth County Surrogate Court, Hall of Records, One East Main Street, Room 114, Freehold, NJ 07728.
- The Proof of Service/Mailing for all interested parties, along with the USPS Return Receipt Form and the Return Receipt Card (commonly known as Green Cards) and/or tracking information.
- The original Affidavit of Personal Service and Notice of Pending Hearing previously completed, served personally and read to the alleged incapacitated person must be returned along with the above Proof of Mailing.
