HOW TO FILE YOUR CASE
The application process involves several steps, including mailing instructions. Please consult the CHECKLIST REMINDER (Person or Person/Estate) and you must consult NJ Courts Guardianship Self-Help website "Steps to Apply."
The statutory filing fee of $200 is to be included when filing your pleadings. Please make sure to include a check that is payable to Monmouth County Surrogate, as cash payments are not accepted.
Please send:
1. One (1) original set of all the forms required.
2. Two (2) duplicate copies of all the forms required.
3. A $200 check for the filing fee payable to Monmouth County Surrogate.
4. Include a self-addressed, postage-paid envelope. It must be large enough to accommodate the duplicate copy of all the forms so that the court will be able to return the filed forms back to you. They will be returned to you with the executed Order Fixing Hearing (Form F).
5. Mail or deliver your forms to:

Keep copies of all papers you provide to the court or any other party. Retain copies of all completed forms and any canceled checks, money orders, receipts, bills, contact estimates, letters, leases, photographs and other important papers that release to your case.
