GUARDIANSHIP REPORTING

As a guardian, you are required to file reports as specified in the executed Superior Court Judgement (FORM G). Guardians should expect to receive an annual reporting reminder in the mail from the Surrogate's Court. The reminder will contain your docket number, reporting range, reporting due date, and the types of reports you need to submit. Even if you don't receive a reminder. you are still responsible for the timely filing of all guardianship reporting forms.

RESOURCES, REPORTS AND FORMS
Guardians must file periodic reports with the court, as specified in the written Judgment (Form G) regarding reporting schedules and required forms. Reports must be submitted annually within 14 days of the anniversary of your Judgment date. Your Judgment and reminder letter will specify which reports you need to complete.
