Dental Plans

Monmouth County offers two types of dental plans.  Both plans are 100% employee funded with pre-tax dollars. Once enrolled, participation must be for a full twelve (12) month plan year, March through February.  If you choose not to enroll upon commencing employment, you are not eligible to enroll for 18 months from your benefits effective date or the next open enrollment period which ever is longer. NOTE: Orthodontic benefits are not covered under either program.

Horizon Dental Choice (HDC) 

Operates much like a medical HMO, with members required to pre-select from a limited number of providers. There is no deductible and has unlimited annual benefits. Coverage is as follows:  Preventive 100%, Basic 100%, Major 50%.  HDC is less costly than HDOP.

Horizon Dental Option Plan (HDOP)

Similar to a traditional plan, which allows members to obtain services from the dentist of their choice.  There is a deductible of $25 per person/$75 per family and an annual benefit maximum of $2,000 per family member.  Coverage is as follows:
Preventive 100% of the maximum allowable charge (MAC),
Basic 80% MAC,
Major 50% of MAC and the employee must be enrolled at least 6 months.

Dental Services Organization (DSO) 

All services under this plan are available at Eastern Dental, a provider of dental care for Dental Services Organization, Inc., a licensed Dental Provider Organization in the State of New Jersey. Services are provided in a dental clinic setting and advance selection of a participating clinic is required. This is the only plan available through payroll deductions that offers orthodontic care. Coverage is: Preventive 100%, Basic 100%, No deductibles and unlimited annual benefits.

Vision Plan

Horizon offers Monmouth County employees vision coverage in association with Davis Vision. This plan is in addition to the eye care provisions under the medical plans and is designed to give you access to discount prices on eyewear at over 60,000 points of access nationwide.

Deferred Compensation Program (457B)

This plan is provided by Nationwide Retirement Systems and is similar to a 401K program. You can contribute as little as $10 per pay up to a maximum of $15,500 per year in most cases. Your contribution to this account is tax deferred. Because your gross salary is reduced by the amount of your contribution, your taxable income is lowered. Roll over from most IRAs is accepted.
 
This plan is provided by Nationwide Retirement Plans and is similar to a 401K program. You can contribute as little as $25 per pay. The total annual deferral amount to all 457 plans for 2017 is the lesser of $18,000 ($24,000 with the Over Age 50 Catch Up option or $36,000 with the special 457(b) Three-Year Catch Up option) or 100% of includible compensation. Your contribution to this account is tax deferred. Because your gross salary is reduced by the amount of your contribution, your taxable income is lowered. Rollovers are accepted from most traditional IRAs, 401k’s, 403(b)’s, and other 457(b)’s.

Flexible Spending Account

Medical Flexible Spending Account

This plan is administered through Insurance Administrator of America (IAA). A Flexible Spending Account (FSA) is a great way to pay for out-of-pocket medical expenses and associated costs. Each pay period, the amount you designated at open enrollment will be deducted from your paycheck and deposited into this account. To receive reimbursement, you simply need to complete and submit a Reimbursement Request Form with receipts to IAA. Flexible Spending Accounts allow you to save money because contributions are made from your paycheck before taxes.
 
A medical Flexible Spending Account helps you pay for eligible health care expenses that are not covered or reimbursed by any other insurance. The maximum amount you can contribute is $2,640 ($110 per pay) for a plan year and the minimum is $240 ($10 per pay) per plan year.

Dependent Care Assistance Program

A Dependent Care Assistance Program is administered through the Finance Department and allows you to be reimbursed on a pre-tax basis for child care or adult dependent care expenses for qualified dependents that are necessary to allow you or your spouse to work, look for work, or attend school full-time. Your account can be used to reimburse you with pre-tax dollars if the expenses for dependents meet the IRS definition of dependent for income tax purposes. An adult (e.g., parent, grandparent, and adult disabled child) may qualify as a dependent if you are providing more than half of that person's support for the year.  Click on the following for the more information: Dependent Care Assistance Plan Employee Manual
 
Short Term Life Insurance 

This plan is administered by Colonial Life on an after tax basis. This disability insurance does not replace the disability insurance that you may be eligible for from the County at no cost to you, however, it does run concurrent. This insurance is a “guaranteed” issue for new employees. It is available during open enrollment. The amount of monthly insurance you can sign up for is a percent of your monthly salary up to a maximum benefit of $5,000 per month. There is a minimum fourteen (14) day waiting or elimination period before you can begin to receive the monthly benefit for up to one year. Contribution is based on salary and age.

Whole Life Insurance 

This plan is administered by Colonial Life on an after tax basis. You can select the benefit amount that meets your specific needs and this plan is portable. Contribution is based on salary and age; therefore, you should speak with a representative regarding interest in this plan. The plan has a guaranteed cash accumulation. Guaranteed issue up to a $100,000 face amount. You may purchase additional coverage for dependents.

Universal Life Insurance

This plan is administered by Winston Financial Services and has an optional Long Term Care Benefit. This plan is offered on an after tax basis and is only available to employees who enrolled with Winston prior to 2017.
