ORDER FIXING HEARING (FORM F) AND YOUR COPIES ONCE RECEIVED

Important information to remember after receiving the Judge - signed Order Fixing Hearing (Form F)

You must serve all parties involved in the filed pleadings by Certified and Regular Mail.

1. Hand-serve and read to the Alleged Incapacitated Person:

• NOTICE OF PENDING HEARING
• AFFIDAVIT OF PERSONAL SERVICE
• A copy of all filed papers is required to be given to that person
• All documents need to be filled out and completed

2. Serve the filed documents listed below via USPS Certified Mail (with green return receipt card) to Court Appointed Attorney, County Adjuster, (if you checked the box on the CIS-Form A indicating case type as Title 30 then the DDD must be served) and to all interested parties, listed in Form A.

• CIS (Form A)
• Verified Complaint (Form B)
• All Certifications - Doctors (Form D) and Assets (Form C)
• Order Fixed Hearing (Form F)
• Proposed Judgment (Form G) - Person / Person & Estate
